Telehealth consent

By booking a video visit you consent to receive care by telehealth. Telehealth has limits: your provider cannot examine you in person, and some conditions require an in-person visit, labs or imaging.

Video visits use a secure platform. You are responsible for being in a private location with a reliable connection at your appointment time.

You may stop a telehealth visit at any time and request in-person care. Your provider may also decide that in-person care is needed.

Information shared during your visit is part of your medical record and protected under applicable privacy law.
